You will assess learning barriers, set goals, implement evidence-based interventions, and measure outcomes within a comprehensive glycemic management program. In this role you will liaise with physicians and the care team, maintain up-to-date education materials, and coach staff to deliver best-practice diabetes education consistent with ADA #J-18808-Ljbffr
