Job Description

The Case Management Registered Nurse plays a crucial role in patient care, collaborating with healthcare providers to assess, plan, implement, and evaluate care plans in alignment with patients' needs and hospital guidelines.

Responsibilities

• Conduct thorough patient assessments and evaluations.
• Develop and manage care plans, ensuring integration of services.
• Collaborate with interdisciplinary teams to provide comprehensive care.
• Monitor patient progress and adjust care plans as necessary.
• Provide education and support to patients and families regarding care processes.

Required Experience / Certifications / Licensure

• Active Registered Nurse (RN) license in the State of Florida.
• Experience in acute care and case management preferred.
• Familiarity with Meditech and Midas software is advantageous.
