Physical Therapist Homecare, Anne Arundel County

Provides evaluation and physical therapy treatment in accordance with agency standards and the laws and regulations governing the provision of physical therapy services in Maryland, the District of Columbia, or Virginia. Primary Duties and Responsibilities Analyzes and documents patients and family's response to interventions, evaluates measurable progress toward goals, and revises the plan of care using knowledge of interventions and resources. Establishes and documents a teaching plan for patients/families based on their level of knowledge, diagnosis, prescribed treatment, and available resources, and communicates respectfully with patients/families, supervisor, peers, and other health team members. Contributes to the achievement of established goals and objectives and adheres to department policies, procedures, quality standards, and safety standards; completes annual Code of Conduct compliance and other mandatory training. Implements an effective physical therapy treatment plan in conjunction with the patient, family/caregiver, physician, and other members of the health care team, modifying the plan as needed. Evaluates the patient through interview, observation, palpation, and specialized assessments including goniometry, muscle strength testing, and sensory testing. Independently implements and documents a plan of care for patients ranging from routine to complex problems to facilitate continuity of care, ensuring interventions reflect the standard of care and utilize agency and community resources. Organizes patient assignments for timely task completion and seeks assistance with complex patient problems; maintains clinical records, collects data, and prepares activity reports, submitting admission records within 48 hours and daily records within 24 hours with at least 95% technical audit accuracy. Initiates and participates in regularly scheduled case conferences involving all disciplines, including patient and family in mutual goal setting and care plan revision. Maintains effective working relationships with other departments and participates in multidisciplinary quality and service improvement teams, representing the department and agency in community outreach efforts. Performs history and assessment related to the episode of patient illness to determine habilitation/rehabilitation needs, recommending adaptive equipment and home modifications to maximize functional abilities and safety. Performs procedures and modalities such as therapeutic exercise, functional mobility training, gait training, heat/cold application, and ultrasound. Minimal Qualifications Education Bachelor's degree in Physical Therapy from an accredited school required Experience 1–2 years clinical experience required Licenses and Certifications PT – Physical Therapist License to practice Physical Therapy in the District of Columbia, Maryland, or Virginia required upon hire DL – Valid State Driver's License and dependable vehicle for transportation required upon hire CPR – Cardiac Pulmonary Resuscitation (including BLS and NRP) from either the American Heart Association or American Red Cross required within 90 days Additional unit/specialty certifications may vary by department or business unit Knowledge, Skills and Abilities Good clinical skills Organizational skills Excellent verbal and written communication skills Knowledge of home equipment, home care reimbursement, and regulations governing PTAs Basic computer skills preferred Smartphone experience This position has a hiring range of USD $72,758.00 – USD $130,041.00 per year. #J-18808-Ljbffr MedStar Health
