APhT - Oncology Pharmacy Technician Specialist

Summary This role involves obtaining and updating medication histories for patients admitted or pending admission, ensuring accurate medication reconciliation within the electronic medical record system. The specialist will prioritize medication histories, gather comprehensive medication information from multiple sources, and document details including drug name, dose, route, frequency, and last dose taken. Key Responsibilities Navigate electronic medical records to complete medication histories for admitted or pending patients. Collect medication information through patient interviews, physician offices, pharmacies, and other healthcare facilities. Document medication and allergy information accurately and promptly. Refer unresolved medication issues to pharmacists and admitting providers. Maintain patient confidentiality and secure sensitive information. Report safety hazards to supervisors in a timely manner. Conduct educational sessions for technicians, nurses, and other healthcare staff, including patient education. Precept and train pharmacy technicians, students, pharmacists, and nursing staff on medication history processes. Assist with data collection for quality improvement initiatives. Education Pharmacy Technician Certification Board (PTCB) certification required unless currently enrolled in a PharmD program. Registration with the state Board of Pharmacy is mandatory. Experience Minimum of 6 months to 1 year as a pharmacy technician required; 2 years of retail pharmacy experience preferred. Skills and Abilities Strong attention to detail and technical proficiency, knowledge of common medication doses and forms preferred. Excellent interpersonal and communication skills to interact effectively with patients and healthcare professionals. Ability to prioritize tasks in a high-volume environment and adapt to changing workloads. Physical Requirements Ability to work independently in a fast-paced setting, read physician notes and medication labels, and operate pharmacy equipment.
