Hourly Pay Rate: $67

*Rate includes $202 daily tax-exempt stipend

Requirements:

• Active RN license in the State of New York
• UR/CDI Experience, CDI Certification Preferred

Responsibilities:

• Perform comprehensive assessments of patients' medical, psychosocial and discharge needs.
• Advocate for patient needs and appropriate resource utilization.
• Develop, implement and monitor individualized care plans to facilitate safe, timely and effective patient transitions across care settings.
