Job Title: Behavioral Health Care Coordinator

Location: Hybrid (Travel may be required to Joliet & Oswego, IL)

Contract Duration : 3 Months

Work Hours: Monday to Friday | 8:00 AM – 5:00 PM

Key Responsibilities:

• Conduct health assessments and develop individualized care plans
• Coordinate healthcare services, transitions of care, and community resource referrals
• Educate members on disease management, wellness programs, benefits, and available resources
• Monitor member progress and update care plans as needed
• Collaborate with providers, caregivers, and interdisciplinary teams
• Address member inquiries and resolve care-related issues
• Maintain accurate documentation and meet productivity standards
• Support audit readiness, quality initiatives, and special projects
• Ensure compliance with HIPAA, NCQA/URAC standards, and state regulations

Qualifications:

• Current, unrestricted RN, LPC, LMSW, LCPC, LCSW, LMFT, LPCC, LISW, LMHC, or equivalent license in the state of operation.
• Minimum 2 years of direct clinical experience.
• 3+ years of managed care, wellness, case management, or care coordination experience preferred
• Strong care planning, health education, and medical management knowledge
• Excellent communication, customer service, and relationship-building skills
• Proficiency with Microsoft Office, databases, and web-based applications
• Ability to travel within assigned territory as needed

Preferred Qualifications

• Experience managing complex, catastrophic, or high-risk populations
• Condition management and patient education experience
• Government programs and population health management experience
• Case Management Certification (CCM) or other healthcare certification
• Bilingual English/Spanish preferred
