Occupational Therapist Assistant

We are seeking a Certified Occupational Therapy Assistant (COTA) to join our growing home health therapy team . This is a rewarding opportunity to deliver one-on-one patient care in the home setting , helping patients regain independence, improve mobility, and restore daily function after illness, injury, or surgery.
If you are passionate about rehabilitation, patient-centered care, and meaningful outcomes , this role is for you. What You'll Do- Provide occupational therapy treatments in home health settings under the direction of an Occupational Therapist
- Support patients with activities of daily living (ADLs) such as dressing, bathing, grooming, and mobility
- Assist in improving strength, coordination, balance, and functional independence
- Deliver care focused on rehabilitation, recovery, and quality of life improvement
- Follow infection control, safety, and universal precautions
- Maintain accurate and timely clinical documentation
- Communicate effectively with patients, families, and interdisciplinary care teams
- Active Certified Occupational Therapy Assistant (COTA) license
- Current CPR certification
- Valid driver's license and reliable transportation
- Home health or skilled nursing facility experience preferred
- Experience with HomeCare HomeBase (HCHB) preferred but not required
- Ability to perform hands-on patient care in home environments
- Must be able to stand, walk, squat, kneel, crawl, and climb as needed
- Ability to lift, carry, push, or pull up to 100 lbs
- Flexible, independent home health work environment
- Strong focus on patient outcomes and meaningful recovery
