PACE Health Center Manager Registered Nurse

Overview Under the supervision of the PACE Director and under the direction of primary care, this position is responsible for assessing, coordinating, monitoring, and providing clinical treatment and services required by PACE members. Responsibilities PERFORMANCE AREA 1: Works closely with the Interdisciplinary Team to:
- Receives and communicates appropriate information to staff and keeps PACE Director informed of changes, needs, complaints, etc.
- Responds to emergencies involving participants.
- Participates in the Quality Assurance program by conducting audits and submitting results in writing.
- Direct, supervise and evaluate the performance of the clinic team, and maintain a matrix reporting/supervisory relationship with staff involved in participant care.
- Interviews, recommends for hire, and trains staff specifically for Clinic Operations.
- Prepare periodic reports and assist with QI projects to review staff performance as related to planning, quality of work, procedures, and participant care.
- In collaboration with other site managers and supervisors, establish and maintain uniform, consistent delivery of care and services for participants, including internal and external appointments with transportation needs.
- Arrange Licensed Vocational Nursing (LVN) and Medical Assistant (MA) coverage for all areas of responsibility.
- Work with supervisory staff and site director to assure that cultural needs of participants are met in the day center.
- Assists in the development and implementation of new procedures.
- Performs functions related to clinical assessment, care planning, evaluation of participant care.
- Teaches participants and families based on identified needs of participant.
- Documents nursing care in the medical record.
- Able to triage participants in clinic effectively.
- On call responsibilities.
- Administers medications and maintains medication system including over the counter, contingency and emergency medication supply participants under clinic standards of practice and within scope of licensure.
- Completes transcription and follow-up of medical orders.
- Performs phlebotomy procedures as needed.
- Maintains cleanliness and organization in the clinic, especially in the medication area and cart.
- Participates and functions as a member of the interdisciplinary team and rotates as needed.
- Independently delivers nursing care and services to program members within scope of licensure and per primary care orders or nursing care plan.
- Schedules and coordinates med pass and monitor for quality review of medication errors.
- Mentors and assists in the orientation of new clinic staff hires.
- In collaboration with the purchasing agent, oversees medical, pharmaceutical and oxygen supplies in the clinic. Tracks assignment of oxygen concentrators and other Durable Medical Equipment as indicated.
- Scheduling and managing appointments/assessments for MDs and Nurse Practitioners
- Wound care and maintaining wound records/documentation
- Daily nursing tasks, preparing clinic for patients to be seen by provider.
- Assist with bedside procedure including gynecological exams, phlebotomy.
- Maintaining participant safety while at the center
- Hold formal monthly and as needed meetings with PCWs and records minutes.
- Monitor transportation services and resolves problems as necessary with Transportation Coordinator.
- Consult and collaborate with the Dietitian and Recreation Therapist on matters related to food/nutritional services within the Adult Day Health Center.
- In consultation with other management staff, develop and implement policies, procedures and standards for Health Center. Communicate changes to staff, monitor outcomes and makes changes as needed.
- Assume a leadership role in the development, implementation and refinement of day center systems and coordination of schedules which involves rigorous communication and collaboration with all departments, including transportation, the clinic, and rehabilitation therapy, recreation therapy, dining services, home care, social work, maintenance and environmental services.
- Interface with program participants, families, contracted providers and the public in regard to issues related to the routine operation of the Health Center.
- Monitor the physical environment of the Health Center in conjunction with assigned staff, to ensure a safe, orderly, and sanitary environment is consistently maintained for participants and staff. Notifies appropriate maintenance or housekeeping staff of problems or concerns as indicated; consults with Program Director regarding significant repair/replacement needs.
- Review safety issues, including participant falls and incidents as they occur, and reports to appropriate personnel, and appropriate committees.
- Serve on assigned committees of the organization and regularly attends meetings of same.
- Assist the PACE Director with budgeting, monitoring and analyzing site performance trends in utilization, financial and participant outcomes.
- Collaborate with PACE Director to investigate and resolve grievances, service delivery concerns, and incidents relating to Health Center operations.
- Supervise the Health Center staff.
- Act as liaison between providers/family/pharmacy and will be an active member of interdisciplinary team and cross train to home care RN position.
- Ability to maintain excellent and respectful working relationship with co-workers and customers from diverse backgrounds.

- Attends and actively participates in all meetings (e.g., department meetings, program meetings, employee staff meetings) and other activities as required or assigned.
- Attends workshops/seminars as necessary to increase skills and knowledge to provide effective care, treatment, and/or leadership.
- Supports the overall needs of the health center by working flexible or extended hours when necessary.
- Supports the needs of the health center by traveling to other UHC clinics when staffing needs dictate the need for licensed personnel.
- Demonstrates awareness of, and compliance with organizational mission and objective of UHC to provide health care access and support services for all members of the community.
- Supports their own staff development by completing the required hours of continuing education each year as well as new certifications as required.
- Other work-related duties as assigned by supervisor. Duties and responsibilities may be added, deleted, or changed at any time at the discretion of management, formally or informally either verbally or in writing.
- Maintains confidentiality and respect for information regarding patients and other team members; abides by UHC Rules of Confidentiality and general HIPAA regulations regarding privacy.
- Displays a positive, professional and respectful demeanor at all times toward employees, peers, professional contacts, and patients served, maintaining a professional appearance and positive image for the health centers.
- Contributes to the team by promoting positive staff interaction, maintains open communication with other programs/departments.
- Bachelor's Degree
- Licensed as a Registered Nurse in the state of employment
- BLS certification required.
- One-year experience as a Registered Nurse; experience with an elderly population preferred
- Bilingual (English/Spanish) preferred
- Able to quickly build and maintain rapport with patients and providers of differing backgrounds; team player
- Must be able to read, write, speak and understand the English language.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to deal tactfully with staff, members, visitors, government agencies/personnel and the general public.
- Must be knowledgeable of dietary procedures.
- Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult members.
- Productive as both an independent and interdisciplinary team contributor.
- Effective organizational skills.
- Demonstrates necessary skills and knowledge as needed for position-specific Competency Assessment Profile.
- Requires valid driver's license.
- Experience/familiarity with PACE program and Electronic Medical Records systems.
- Previous Home Health experience
- Ability to sit and/or stand for long periods of time
- Ability to reach, bend, climb, stoop and lift and carry up to 20 lbs of medical/office supplies for routine performance of essential job functions
- Emergency response capabilities for possible crisis situations
- Must have annual TB test &/or complete TB questionnaire annually
- Manual dexterity and strength sufficient to enter information via computer keyboard for long periods of time, to write notes and information as needed, and to pick up and hold paperwork, supplies and other items
- Speaking and hearing ability sufficient to handle phone and in-person conversations with patients/clients, clinicians and other staff and to listen to breathing, pulse and circulation, as part of patient examination and treatment
- Visual acuity and reading ability sufficient to effectively read records and documents, and to accurately view information on a computer monitor
- Normal tactile feeling required to perform duties, including taking patient's pulse.
- Eye/hand coordination, hearing and visual acuity necessary for driving safely
