Position Title: Community Health Registered NursePosition Goal:Community Health Registered nurses (CHRN) at REACH Medical, PLLC work to ensure that all patients and their family/support systems receive the highest quality health care. Our practice aims to consistently meet the goals of creating a safe, inclusive, professional, and friendly work environment for staff, patients, and clinical providers.Role:The primary duties of the CHRN at REACH Medical will be to carry out all nursing clinical, social determinants of health, and administrative tasks required to ensure great patient care occurs without stigma in our low threshold harm reduction medical practice. The CHRN will have much autonomy in terms of several of the harm reduction programs including patient centered recovery plans, medication for opiate use disorder (MOUD), PrEP, PEP, and Hep C testing and treatment options. The CHRN will therefore need to gain extensive knowledge in these areas of patient care.The CHRN will be responsible, along with all other staff, to be sure patients are scheduled for the appropriate visit type and transportation is scheduled, if necessary. The CHRN will ensure that the rooms are stocked and ready for patients to be seen. The CHRN will make sure patients are triaged, along with assuring patient intakes and screenings for social determinants of health, behavioral health, court case involvement, and food security have been done appropriately. The CHRN will ensure that the medical provider sees the patient in a timely manner and will also ensure that the discharge of the patient from the visit is done appropriately.Responsibilities:Develop patient centered recovery plans.Raise awareness of existing social and other support services and facilitate referrals and warm hand offs when appropriate.Provide non-clinical crisis support, especially after periods of hospitalization or incarceration that may include linkage to clinical specialists or case management services at REACH.Overdose response.Work with participants to identify strengths.Educate patients and offer linkage to care to the various forms of recovery including all forms of medications for addiction treatment inpatient care and detox.Understand and be cross-trained in all nursing clinical and peer relates aspects of patient care.Practice within Health Care Regulations and REACH Medical Policies and ProceduresWork ScheduleBe available for day time hours and or evening and weekend hoursBe available answer an on-call phone at agreed upon times if an when REACH Medical puts an on-call system in for off hours.Use HIPAA compliant texting software assigned by the practice in order to communicate about prescriptions or other patient information with the patient.Patient ScheduleHelp ensure that patients are scheduled for the appropriate appointments.Help ensure that walk- ins are seen in a timely manner and that all medical, non-medical, and social needs are assessed.TriageEnsure that triage is done appropriately with vital signs and the appropriate screenings of each patient.Medication for Opioid Use DisorderEnsure that the Patient Monitoring Program has been checked for all ORT prescriptions (NYS PMP).Establish and monitor each medical provider’s ORT list to be sure the practice is operating within state federal guidelines.Ensure all patient intakes are done appropriately.Ensure COW scores are done appropriately, when applicable.Recommend and work with medical providers and the patient to find the appropriate dose of MOUD for patient.Hepatitis C Treatment, PReP, and PEPBecome familiar with the practice protocols for these medications.Be able to communicate the value of these medications to patients in a non- stigmatizing judgmental way.QualificationsExtensive understanding of harm reduction and low threshold care.Lived experience preferred.3-5 years of experience in health care.Licensed as a Registered Nurse in the state of New York.Must possess strong intrapersonal skills with a commitment to provide respectful, culturally-competent communication to fit the context of our target population.Ability to work evenings and weekends as required.Exceptional communication skills and organization skills including the ability to develop and implement meetings, events, action plans and project/programs.Benefits:The position offers a competitive salary and benefitsREACH is dedicated to health equity; we are committed to hiring and developing well-qualified staff who reflect racial, ethnic, social and cultural diversity of the people we wish to serve.
