Details Long Term Care / Skilled Nursing Facility setting Day shift schedule, Monday through Friday 8-hour shifts with flexible start times between 6 AM and 10 AM, as coordinated with the Director of Rehabilitation No weekend requirements No on-call requirements 13-week contract assignment Requirements Current OT license required (license required if applicable) Minimum 1 year of OT experience required LTC or SNF setting experience required Adherence to NBCOT/AOTA and state practice acts and code of ethics 1 professional reference required for consideration Additional Information Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care Utilize standardized, evidence-based assessments to guide clinical interventions Provide rehabilitative treatment including ADL retraining, therapeutic activities/exercises, modalities, and assistive devices Assess patient progress, adjust treatment plans, and document per regulatory and clinical requirements Provide consultation and counseling to patients, families, and caregivers Communicate with supervisors and interdisciplinary team regarding patient progress and treatment plans Complete comprehensive discharge summaries and maintain accurate treatment records Obtain necessary DME or supplies as needed for patient care Supervise occupational therapy assistants, rehab aides, and/or students as needed Participate in required facility meetings, trainings, and in-services No more than 5 days of requested RTO permitted during the assignment American Traveler Job ID P-747587.
