Direct Support StaffSupport our individuals with all of their needs. Ensure their health & safety needs. Assist them with their career goals, be a mentor and example to all.Major Duties and Responsibilities:Provide training, supervision, instruction and supports to individuals in all agency settings, on site or in the community.Ensure that you are aware where your assigned individuals are during the day, let another staff know that you are leaving the floor and ensure that they have taken responsibility for your individuals until you get back.Attend agency required trainings and meetings and give input on individual plan development.Assist to develop and track all worker goals and other daily documentationBe aware of all Mealtime Procedures and always supervise individuals during mealsProvide transportation as needed for supported employment opportunitiesProvide on-going necessary supports as indicated by their IP for each worker. Assist them to complete the tasks as independently as possible assisting if needed for the task to be completed properlyAssist staff & individuals during fire drills or other emergency situationsPass medications if applicableAssist in providing a positive program environment to staff & participantsComplete and maintain all daily individual documentation and worker weekly notes.At the end of each month remove all data collection sheets and weekly notes from each person's book that you are assigned that day.Ensure completion of all entries and turn into the Assistant ManagerEnsure all extra documents are filled out by end of day if applicable, i.e.: 255, Seizure protocol, behavior data, ABC's, minor incidents, etc.At the beginning of each month ensure that the new month's data collection pages and weekly notes are put into the books that you are assigned that daySubmit your COMPLETED timesheets and mileage forms to your manager by Friday PMSubmit your Auto Insurance Declaration page to the Administrative Assistant upon renewal or every 6monthsDocument appropriately in all Travel Books when using the Company VehiclesIt is your responsibility to let your immediate supervisor know if any of your trainings are getting ready to expire. Please give them 60 days' notice of expirationBe on time & in the common area until the individuals start their shiftStay with your individuals at the end of the day until they are out of the buildingBe a team playerLearn & follow all policy & procedures of the agencyLearn and follow all safety & emergency procedures for the agencyBe involved in OWC fundraising eventsPerform other duties as assigned, or as they arise.Assist individuals with toileting and hygiene issues as indicated in Level of NeedQualifications for the Job:GED/High School diplomaAny CNA experience is a plus3 years' experience in a human service setting (preferred)Must be reliable, dependable, flexible with your hoursMust possess proven experience and successCPR/First Aid, Med Cert, PMT a plusMust possess valid driver's license and have 5 years' experience clean drivingMust be able to work as part of a team as well as be independentKey Competencies:Computer skills, Organizational skills, Interpersonal and Communication skills, Problem solving skills, good verbal and written skills, caring and nurturing, honest, reliable and punctual**No breaks are given in this postion as you are responsible for the safety and well being of individuals**Physical Requirements:Must be able to lift & carry a minimum of 40 pounds
