Hospice RN Case Manager — Coordinate Care that Matters
Transitions Care
Transitions Hospice is seeking an RN Case Manager to serve as the clinical anchor for patients and families, coordinating care and guiding the interdisciplinary team. You will assess needs, develop personalized care plans with physicians, administer medications per orders, and ensure accurate documentation while educating families for confidence and support. You will participate in on-call rotation and work in a home-based environment with a patient-centered focus on dignity and comfort at the #J-18808-Ljbffr Transitions Care
