PRN Wellness Nurse

Description The Neighborhood at Cullman is seeking PRN Wellness Nurses to join their team. The Wellness Nurse reports directly to the Wellness Director. Shift Details Mon‑Fri; Weekends, holidays, all shifts to cover when full‑time employees take PTO or call out. Purpose The Wellness Nurse/Nurse Med Tech is responsible for monitoring the health and wellbeing of our residents and providing leadership support and assistance with resident care and services. Responsibilities Identifies ongoing needs and services of residents through the assessment / Personalized Service Plan (PSP) process. Ensures proper follow through and documentation for residents with a change in clinical care needs. Assists the Wellness Director with completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident monthly. Informs the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move‑out. Assists with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates. Assists with direct resident care as needed. Education, Experience, Licensure & Certification Must be a Licensed Practical Nurse or Registered Nurse. A minimum of one year of experience working with elders or disabled individuals, preferred. Must have demonstrated Leadership capabilities. Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred. Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times. Requirements Understanding of infection control procedures. Proficient in using MS Office. Understanding and able to implement Personalized Service Plans. Understanding of advanced directives and end‑of‑life care. Understanding and embracing the assisted living philosophy. Understanding of nursing function in assisted living, quality of care and quality of life objectives. Demonstrated knowledge of state regulations. Proficient in using state audit compliance tools. Proficient in time management skills and adherence to deadlines. Excellent grammar and documentation skills. Understanding of basic concepts of risk management.
