- Developing and implementing individualized, evidence-based treatment plans that support each patient's functional goals and maximize independence.
- Providing skilled therapy interventions for patients with communication, language, cognitive, speech, voice, and swallowing disorders across diverse outpatient populations.
- Collaborating with physicians, occupational therapists, physical therapists, nurses, social workers, and other interdisciplinary team members to coordinate comprehensive patient care.
- Educating patients, families, and caregivers on therapeutic strategies, home exercise programs, compensatory techniques, adaptive equipment, and recommendations that support continued progress outside of therapy sessions.
- Developing appropriate discharge plans and home programs that promote continued functional improvement and successful transition following therapy.
- Completing timely and accurate clinical documentation, including evaluations, treatment notes, progress reports, discharge summaries, billing documentation, and other required medical records in accordance with RWJBarnabas Health policies and professional practice standards.
- Accurately documenting therapy services and associated charges in accordance with departmental policies and regulatory requirements.
- Recommending adaptive equipment and providing patient and caregiver education to maximize safety, communication, swallowing function, and independence.
- Supervising speech-language pathology students and clinical fellows, when applicable, by providing mentorship, constructive feedback, and formal performance evaluations.
- Assisting with the maintenance, organization, and recommendations for departmental treatment equipment, materials, and supplies.
- Participating in continuing education, mandatory training, staff meetings, and professional development activities while maintaining clinical competencies.
- Meeting productivity expectations while providing compassionate, patient-centered care and representing RWJBarnabas Health with professionalism and clinical excellence.
This role might be for you if:
- You are passionate about helping patients improve communication, cognition, swallowing, and overall functional independence.
- You enjoy building meaningful relationships with patients and families while providing compassionate, evidence-based care.
- You thrive in a collaborative outpatient rehabilitation environment and enjoy working alongside an interdisciplinary team.
- You communicate effectively with patients, caregivers, physicians, and healthcare professionals across a variety of clinical settings.
- You are organized, adaptable, and able to manage a diverse caseload while maintaining high-quality documentation and meeting productivity expectations.
- You enjoy developing individualized treatment plans that help patients achieve meaningful functional outcomes.
