Primary Care Physician

Primary Care Physician (PCP) Role A PCP demonstrates accountability for outcomes, acts as a health coach, simplifies for action, and serves up to 450 patients. The PCP’s panel metrics include patient admissions per thousand, appointment slots per day, patient experience (CGCAHPS), clinical gaps closure, and medical cost measures. The PCP works closely with managers, medical directors, and care teams. Essential Job Duties / Responsibilities Provide independent care for patients with acute and chronic illnesses in older adults. Take full accountability for patient care and outcomes, seek specialist consultation when needed, and remain responsible for detailed care. Engage with hospitalists when a patient is hospitalized. Conduct assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation. Lead the care team (care promoter, facilitator, coordinator). Collaborate with transitional care teams and other resources for patients unable to come to the office. Assist in center management, cover for other providers, recruit patients, and support recruitment of additional providers. Supervise and collaborate with an Advanced Practice Practitioner (APP) while remaining accountable. Perform other duties as assigned by the manager. Leadership: Continuously develop and improve leadership skills. Quality: Work with teams to achieve high quality outcomes and practice improvement. Influence: Compassionately influence patients, teams, and self. Self‑Care: Maintain physical, mental, social, and spiritual wellness. Technical Proficiency: Proficient in Microsoft Office Suite and related software. Travel: Willingness to travel locally, regionally, and nationally as needed. Language: Fluency in English; other languages may be required. Judgment: Use independent judgment. Pay Range $221,141 – $315,915 Salary. Base compensation may vary based on experience, education, location, and other factors. Potential bonuses or commissions apply.
