Schedule NPPA Shifts: Tues, Thurs, Friday, Sat and Sun: 7a-7p, 11a-11p, 7p-7a. M/W: 7a-7p, 9a-5p, 4:30-12:30a, and 7p-7a. Education / Training Graduate of an accredited Nurse Practitioner program. Graduate of an accredited Physician Assistant program. Clinical Experience New grads need training. License Active NY RN and NY NP License and DEA with NY address and schedules 2, 2N, 3, 3N, 4 & 5 for Nurse Practitioner; NY PA license & DEA with NY address and schedules 2, 2N, 3, 3N, 4 & 5 for Physician Assistant. Board Certification Nurse Practitioner: Active certification through AANP/ANCC in FNP. Physician Assistant: Active certification through NCCPA. Merit Badge ACLS and PALS (both AHA) and ATLS. Immunizations Current Flu (every year) Current PPD or Chest Xray (initial credentialing) Current Physical (initial credentialing) Current N95 Fit Test (every year) Titers for MMR-Hep B-Varicella Case Log Must provide documentation of case logs for the past 2 years. CME/CEU Must provide documentation of CME/CEU for the past two years. Specific Facility Procedures Moderate Sedation. Malpractice Coverage $1M/$3M.
