Company Description Families and Community Together (FACT) is a community-based care management organization serving families and children experiencing serious behavioral health, substance use, or developmental challenges. FACT provides behavioral health management services to all residents of Union County, ensuring access regardless of language, income, or citizenship status. Role Description The Care Manager is a full-time, hybrid role based in Mountainside, NJ, with the ability to complete some responsibilities from home. This role involves assessing the needs of children, youth, and families, developing individualized service plans, and coordinating behavioral health, substance use, and developmental support services. The Care Manager will conduct regular home and community visits, maintain ongoing contact with service providers, and advocate for clients to access appropriate resources. Daily responsibilities include documenting case activity, monitoring progress toward goals, and collaborating with schools, medical professionals, social service agencies, and community partners. The Care Manager will also participate in team meetings, attend trainings, and support community outreach activities to enhance service delivery and awareness. Responsibilities Review referral packets and relevant assessments before the initial meeting with the child and family. Participate in initial family meetings to: Build rapport and trust with youth and caregivers. Introduce the Wraparound process and Individualized Service Plan (ISP). Collaboratively develop a comprehensive crisis plan. Coordinate and schedule ongoing Child and Family Team (CFT) meetings. Proactively support families by: Identifying team members and supports. Linking families to community resources. Updating ISPs and crisis plans as needs change. Ensuring timely access to appropriate formal and informal services. Meet with youth and families at least monthly in the home, community, office, or other appropriate settings. Monitor service delivery, quality of care, and progress toward goals. Complete all required documentation, including contacts, progress notes, and interventions, in CYBER or other designated systems on a weekly basis. Participate in the agency's on-call rotation to provide crisis management support to families. Out-of-Home Treatment Responsibilities When out-of-home treatment is needed, responsibilities include: Completing referral packets and uploading required documents into CYBER. Coordinating with the Specialty Residential Treatment Unit. Facilitating provider and family "meet and greet" visits. Maintaining current records in YouthLink. Coordinating Joint Care Review Plans, when applicable. Additional Responsibilities Complete the Strengths and Needs Assessment (SNA). Maintain annual Care Manager certification requirements. Complete ISP discharge or transfer plans within two (2) business days following the final CFT meeting. Submit flex fund requests and referrals within agency timelines. Requirements Education and Experience Bachelor's degree or higher in Social Work, Psychology, Counseling, or a related human services field required. Minimum of one (1) year of professional experience providing direct services to children and families, preferably in: Care Management Case Management Behavioral Health To qualify for the highest level of the pay range, candidates must have at least ten (10) years of experience within New Jersey's Children's System of Care (CMO). Skills and Qualifications Strong organizational skills with the ability to manage multiple priorities and deadlines. Excellent time management and attention to detail. Strong interpersonal and communication skills with the ability to engage families and collaborate with professional partners. Ability to work independently and as part of a team. Demonstrated cultural competence and sensitivity to diverse family needs. Proficiency with Microsoft Office 365, including: Word Outlook Excel PowerPoint Comfortable using electronic health records and documentation systems, such as CYBER. Valid driver's license in good standing and access to reliable transportation. Bilingual skills are preferred, including but not limited to: Spanish Haitian Creole Portuguese American Sign Language (ASL) Availability for occasional evening and weekend work, including participation in the on-call crisis rotation.
