RN Case Manager
Virtual Vocations Inc
To support kidney health management, the remote RN Case Manager will manage a caseload of members, conduct health screenings, create care plans, and collaborate with healthcare providers while working within Pacific and Mountain time zones. Key responsibilities Handle inbound and outbound calls to provide exceptional service to members Educate members on kidney health, dialysis options, and engage them in HealthMap's Kidney Health Program Coordinate care as a liaison between patients, their support networks, and healthcare providers Required qualifications Active, unrestricted RN license required Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree CCM certification preferred Three (3) years of experience in case management preferred Experience in a dialysis center or transplant center preferred
