LPN

- Ratios: 2 LPNs; 1 on memory care (15 beds) and 1 on skilled (15 beds) and they split assisted (20 beds). Max 25. For days the goal is to have 3 LPNs per shift.
- Required Certifications: BLS, IV certification not required but would be a bonus.
- Common diagnoses/Types of patients: Sisters don't wear identification. Religious name and birth name, they can choose what they go by (this typically is a challenge
coming in). There are 4 Friars and 1 Brother that live there in addition to the Sisters.
- Job duties will include: Administering tube feedings, blood sugars, insulin, vaccinations, PO, SQ, IM medications, skin assessment. Director will do admissions. No
discharges- residents stay for life. Charting is by exception. All levels of care: Independent-SNF and Memory care: CHF, HTN, DM, old strokes, Alz, Dementia, total cares.
- Experience or 'Must have' skills: 1 year of LPN experience. First time travelers are ok. LPNs must be willing to help with hands on care; assisting CNAs. Not just passing
meds. Whatever the need is to provide the best care for the Sisters.
- Common titratable &/or set rate drips: Very rare-- IV abx. Would contract visiting nurses association to bring out IV pump and meds.
- Charge Nurse/House Supervisor? Does the charge nurse take an assignment?: Supervisor and assistant supervisor- they are the RNs in the building. Plan to have a
supervisor on each shift or via overlap. Night shifters can communicate with 2nd shift supervisor or 1st shift supervisor when they are coming/going.
- Are there nurse aids on this unit/Ratios: Yes.
- RT Equipment: O2, Cpap only. No trachs or chronic vents. Breathing tx and inhalers.
- Shifts & Scheduling: All 8s, 6-230; 2-1030; 10-630. Rotating schedule try to follow that but may need to move for PTO or FMLA. Facility asks for flexibility.
- Weekend Requirements: EOW
- Holiday Requirements: If there is a need.
- Floating Requirements: Will be trained on all units and be expected to float throughout the building.
- Scrub Color/Dress Code: Any color of scrubs- solid colors.
- Unit/Department specific orientation: Try for 1 week on memory care and 1 week on Skilled; but may be cut down depending on the candidate.
- Additional Notes: No facial piercing. No offensive tattoos. No face and neck tattoos or cover them. No smoking (tobacco/marajuana) on the premises including the parking
lot. No ear buds or phone in patient care areas. Breaks only.
