Your role will be based at the clinic in Rockport, TX, reporting directly to the Market Chief Medical Officer.

As a Nurse Practitioner (NP) or Physician Assistant (PA) with CenterWell Senior Primary Care , you will collaborate within a multidisciplinary team that includes physicians, pharmacists, care coaches, and behavioral health specialists, all committed to delivering tailored, high-quality care.

Your work will take place onsite at our South Zarzamora clinic in San Antonio, under the supervision of the Associate Medical Director for South San Antonio.

• Deliver comprehensive care to seniors in accordance with clinical standards.
• Engage daily with your care team to ensure coordinated and high-quality service.
• Oversee referrals, coordination with hospitals/SNF, DME, and home health services.
• Accurately document care with assistance from quality-focused coders.
• Participate in on-call rotations and contribute to growth initiatives at your center.
• Demonstrate a commitment to providing value-based care.

• Qualifications We Seek:
  • Master’s Degree in Nursing or completion of a PA program with board certification.
  • Active, unrestricted NP or PA license in the state where you will practice.
  Preferred Qualifications:
  • Experience in value-based care, primary care, or Medicare Advantage environments.
  • Possession of an active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.
  • Familiarity with HEDIS quality indicators and Medicare regulations.

Scheduled Weekly Hours: 40

Compensation Range:

The salary range provided below represents a good faith estimate of the starting base pay for full-time (40 hours per week) employment at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.

$123,800 - $170,300 per year

This position is eligible for a bonus incentive plan based on individual and/or company performance.
