We are seeking a Home Health RN/Case Manager who is passionate and knowledgeable as well as has good time management skills. Home health offers flexibility in your work week. Finally, a position that you do not have to choose from your family and your career. As a RN Case manager in home care, you'll have the privilege of providing skilled nursing care that is customized to the patient's home and daily life. If you are looking for a role with autonomy and variety, where you can demonstrate your knowledge and skills and apply them to the patient's unique needs with a 5-star team, this is the role for you!

QUALIFICATIONS

Education:

Graduate of an accredited nursing program with ana active RN license

Experience:

Minimum of 1 year of geriatric experience preferred

Knowledge & Skills:
• Strong Organizational and self-management skills
• Excellent Communication skills
• Clinical proficiency

Licensure, registration, or certification:
• Basic Cardiac Life Support, RN License, Driver's license and Vehicle with Current Auto Insurance
