Qualifications

• State License License must be in hand at time of application.
• Certifications BLS
• Certifications ASHA (CCC-SLP)
• Specialty Experience 1+ years of experience in Cleft Palate and Craniofacial Anomalies in last 3 years.
• Specialty Experience 1+ years of experience in Pediatrics in last 3 years.
• Specialty Experience 1+ years of experience in NICU in last 3 years.
• Skills Assessment NSL skills assessment with 2.7+ score.
• Skills Assessment PSP skills assessment with 2.7+ score.
• Skills Assessment CPC skills assessment with 2.7+ score.
• Additional Confirmations I have not worked as a permanent employee of the facility in the past six months.
• Additional Confirmations I have not requested more than three holidays off during my assignment.
• Additional Confirmations I have submitted all time off requests. I am aware that I cannot make additional requests after submission.
• Additional Confirmations Outpatient experience is required.
• Additional Confirmations Infant MBSS experience is required.
• Additional Confirmations Hospital experience is required.
• COVID Vaccine Regular testing required. Medical and religious declinations are accepted.
• Flu Vaccine (Current Season) Must be for the current flu season. Medical and religious declinations are accepted. Willing to obtain allowed.
• Travel Contract Must travel at least 50.0 miles to receive a travel stipend. Local candidates are eligible, but the pay package may adjust the rate by a few dollars per hour, will not contain a travel stipend, and will be entirely taxable.
• Requested Time Off Up to 5 days of time off can be requested.
