We are seeking a Board‑Certified or Board‑Eligible Neurology Physician (MD/DO) to provide on‑site neurology consultations for residents in skilled nursing and long‑term care settings. The physician will evaluate and manage complex neurologic disorders while collaborating with Advanced Practice Providers and facility teams. Provide on‑site neurology consultations in SNF/LTC settings Diagnose and manage neurologic conditions including stroke, epilepsy, Parkinson’s disease, dementia, neuropathy, and movement disorders Perform comprehensive neurologic examinations and cognitive assessments Order, interpret, and act upon diagnostic studies including imaging and laboratory tests Manage neurologic medications and treatment plans Supervise and collaborate with Nurse Practitioners and interdisciplinary teams Participate in care planning, quality improvement, and hospital readmission reduction initiatives Ensure accurate and regulatory‑compliant documentation in EMR systems MD or DO degree from an accredited medical school Completion of an accredited Neurology residency Active and unrestricted New York State medical license DEA registration Board Eligible or Board Certified in Neurology Experience in SNF/LTC or post-acute care preferred Essen Health Care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population. #J-18808-Ljbffr Essen Health Care
