Home Health RN (Registered Nurse) Position Summary We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide after-hours and weekend patient care within a 65-mile service area. This role is primarily responsible for on-call visits, symptom management, patient and family education, and delivering high-quality care in the home setting. Schedule After-hours coverage: 4:30 PM to 8:00 AM Weekend coverage: Saturday and Sunday Primary on-call role Coverage Area Up to 65 radial miles Responsibilities Provide after-hours home visits to patients as needed Assess and manage symptoms to ensure patient comfort and safety Perform medication management and education Deliver personal care and nursing interventions Support patients and families with education, guidance, and resources Educate families on disease progression and the dying process Complete admissions and required assessments Conduct symptom follow-up and PRN visits Perform recertification visits and documentation Collaborate with the care team to ensure continuity of care Qualifications Required Active RN (Registered Nurse) license Prior hospice experience Strong clinical assessment and communication skills Ability to work independently in the field Preferred Experience with Homecare Homebase (HCHB) EMR system Excellent patient and family education skills Strong time management and organizational abilities Mileage Reimbursement Mileage reimbursement is provided between patient visits Mileage is not reimbursed from home to the first patient visit or from the last patient visit back home By providing your phone number, you consent to: (1) receive automated text messages and calls from the Judge Group, Inc. and its affiliates (collectively "Judge") to such phone number regarding job opportunities, your job application, and for other related purposes.
