Licensed Practical Nurse (LPN)

LPN PositionThe LPN provides nursing assessments and oversight of caregivers and patients in their homes in accordance with physician orders, agency policies, and applicable federal and Florida state regulations. The LPN is responsible for assessing patient needs, implementing and coordinating care plans, providing patient and family education, and ensuring high-quality, compassionate care while promoting patient safety and independence, and supervision of care staff.Location: Southern Pinellas County (assigned client homes)Employment Type: PRN / Per DiemEssential Duties and Responsibilities:Provide direct, one-on-one nursing care to assigned private duty clients in the homePerform ongoing patient assessments and monitor changes in conditionAdminister medications and treatments as prescribedProvide non-skilled nursing care as appropriate, which may include:Medication monitoringVital sign monitoringChronic condition managementDocumentation using EMRCommunicate patient status and concerns to physicians, agency supervisors, and family membersEducate clients and caregivers on care routines, medications, and safety practicesMaintain accurate and timely documentation of care providedFollow infection control, safety, and HIPAA guidelines at all timesPromote client independence, dignity, and comfortRespond appropriately to emergency situations and follow agency protocolsLead orientation for new caregiversWork full time or close to it to cover other RN vacation timeQualifications:Active Florida Licensed Practical Nurse (LPN) license in good standingMinimum 2 years nursing experience preferred (private duty, home care, or long-term care experience a plus)Current BLS/CPR certificationAbility to work independently with minimal supervisionStrong interpersonal and communication skillsCompassionate, patient-centered approach to careValid Florida driver's license, reliable transportation, and current auto insurancePhysical Requirements:Ability to travel to patient homes within the service areaAbility to stand, walk, bend, and perform nursing tasks in a home environmentWork Environment:Patient homes and community settingsFlexible scheduling based on patient needs and nurse availabilityBenefits:Flexible PRN schedulingCompetitive per-visit or hourly compensationSupportive clinical and administrative teamThis position requires Level 2 background screening through Florida's Care Provider Background Screening Clearinghouse.
