Physical Therapist - Home Health | Colorado | Full-Time

Key Responsibilities

• Conduct comprehensive physical therapy assessments and evaluate patients' medical histories to determine appropriate treatment approaches
• Develop and implement customized treatment plans designed to meet individual rehabilitation goals and patient-specific needs
• Deliver therapeutic exercises, manual therapy techniques, and functional training programs to improve patient mobility, strength, and overall functionality
• Provide patient and family education on injury prevention strategies, wellness practices, home exercise programs, and the rehabilitation process
• Monitor, evaluate, and document patient progress throughout treatment, adjusting rehabilitation plans as needed to optimize outcomes
• Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to ensure comprehensive, coordinated patient care
• Maintain accurate and detailed patient records, clinical documentation, and progress notes in compliance with regulatory standards and healthcare guidelines

Qualifications and Requirements

• Master's degree in Physical Therapy from a CAPTE-accredited physical therapist education program
• Minimum of nine (9) months of professional experience as a licensed Physical Therapist
• Home health or community health physical therapy experience preferred
• Current Physical Therapy license in good standing for the state of Colorado (or ability to obtain)
• Valid CPR/BLS certification required and must be maintained throughout employment
• Valid driver's license with reliable transportation to travel to patient homes and business locations
• Strong clinical assessment and treatment planning skills
• Excellent communication and interpersonal abilities for patient education and team collaboration
• Commitment to maintaining professional standards and continuing education in physical therapy practice

Compensation and Benefits

Annual Salary Range: $101,000 - $112,000 (USD)

Competitive compensation package commensurate with experience and qualifications.
