Therapist - Speech Language Pathologist

Job Summary:

Provide specialized speech-language pathology services to residents, helping improve communication and swallowing functions. Collaborate with healthcare teams to deliver patient-centered care within a skilled nursing setting.

Responsibilities:

• 
  Assess residents speech, language, and swallowing abilities to develop individualized treatment plans.
  
  
• 
  Implement therapy interventions to enhance communication skills and dietary safety.
  
  
• 
  Document patient progress accurately and adjust treatment plans as needed.
  
  
• 
  Educate residents and their families on communication strategies and exercises.
  
  
• 
  Comply with all regulatory and facility standards to ensure quality care.
  
  
• 
  Participate in interdisciplinary team meetings to coordinate care delivery.
  
  

Qualifications:

• 
  Valid state licensure as a Speech Language Pathologist.
  
  
• 
  Recent experience working with adult populations in a skilled nursing facility preferred.
  
  
• 
  Strong evaluation and treatment planning skills.
  
  
• 
  Excellent communication and documentation abilities.
  
  
• 
  Ability to work independently and as part of a team.
  
  
• 
  BLS certification is required or must be obtained.
