Our organization is seeking to fill the position of Registered Dietitian (RD) in Cooperstown, NY .

Key Responsibilities:

• Evaluate and monitor the nutritional health of residents, offering guidance to medical personnel and creating suitable care plans.

• Assess the unique nutritional needs of residents, particularly concerning nutrition support, skin integrity, and significant weight fluctuations.

• Oversee the acquisition of nutritional support supplies and oral supplements in collaboration with external vendors and relevant department heads.

• Ensure that meal services for residents adhere to prescribed dietary modifications.

• Provide education to residents, their families, and staff regarding nutrition and dietary adjustments.

• Regularly review Dietary Care plans to assess their effectiveness and maintain high standards of care.

• Participate in weekly meetings with the interdisciplinary team to evaluate and modify residents’ care plans.

• Collaborate closely with the Speech-Language Pathologist to ensure that residents with dysphagia receive appropriate mechanically altered diets.

• Review all menu modifications to ensure compliance with facility policies and State/Federal regulations.

• Daily update diet orders and menu changes in the computer menu software.

• Supervise and work alongside the dietary team to deliver exceptional care to residents.

• Conduct regular audits in areas pertinent to providing quality nutritional care.

• Ensure adherence to regulations and policies regarding weight monitoring within the facility.

• Engage in daily communication with the interdisciplinary team to ensure quality care for residents.

Qualifications:

• Must hold a Registered Dietitian credential.

• Degree in Nutrition or Dietetics is required.

• Outstanding interpersonal and leadership abilities.

• Familiarity with relevant state and federal regulations.

• Proficient in computer skills, including EMR software and computer-based menu systems.
