Physical Therapist

We are looking for a Physical Therapist to provide one-on-one therapy care for patients in their home setting. Primary Job Responsibilities: Assess a person's condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence. Collaborate with interdisciplinary team and colleagues to ensure a person's needs are met. Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests.Utilize a variety of therapeutic modalities to maximize care provided to clientsRequirements: Vermont PT licensureHome Health experience and OASIS experience a must, as this position will case manageMcKesson/Netsmart EMR experience requiredCOVID VACCINATION REQUIRED (CMS guidelines). No Exemptions accepted.
Shift: DAYS: Monday-Friday (w/ weekend rotation approximately every 4th week)
General Certifications: General Certifications(ACLS,BLS/BCLS,PALS,NRP); Rehabilitation Certifications(ASHA,CHT,ACP Training )
