Pharmacist

Under administrative direction, provides pharmacy medication dispensing and consulting services regarding the effective usage of medications and awareness of drug interactions. Major Duties, Responsibilities Performs and/ or completes assigned pharmaceutical care and administrative duties supporting pharmacy operations within established time frames, meeting established rates of performance for the quality and quantity of work for the position. Serves as a medication information resource to patients, providers, and staff by providing counseling, drug information, drug therapy consultation, health promotion, and disease prevention related to medication use. Communicates with and verifies a patient's understanding of medications and desired outcomes of drug therapy. May be required to provide immunization services in accordance with Centers for Disease Control (CDC) guidelines as directed by the organization; accesses and maintains immunization records through the Arizona State Immunization Information System (ASIIS); oversees proper vaccine storage and handling. Willingness to obtain immunization certification if required by the organization. Responsible for performing final verification of prescriptions and medications being dispensed.Evaluates appropriate drug dosing, drug/food interactions, appropriateness of drug therapy, effective patient counseling, clinical intervention, medication therapy management, and decisions relating to generic and therapeutic substitutions. Reviews and ensures the appropriateness of drug therapy. Maintains a comprehensive understanding that ensures compliance with Federal, State, 340B Drug Pricing Program regulations, and third-party funding sources standards and laws. Protects patient health information and ensures compliance with all HIPAA and corporate compliance standards. Provides leadership, training, operational oversight, and direction of assigned pharmacy staff, ensuring delivery of the highest quality patient care. Performs other job-related duties as assigned. Minimum Qualifications Education: PharmD degree from an accredited educational institution. Experience: A minimum of three (3) years’ experience in a clinical setting (or completion of a clinical residency program) to include a minimum of one (1) year of supervisory experience. Licenses or Certification: Must possess a Pharmacist license from Arizona State Board of Pharmacy. Must possess Basic Life Support Certification and Level I Fingerprint Clearance Card prior to employment. Additional Requirements: Must possess a valid Arizona driver’s license Must possess a current fingerprint clearance card Issued by the Arizona Department of Public Safety. Other Skills Knowledge of: Federally Qualified Community Health Centers Electronic health records documentation and navigation. Business English, spelling, grammar, punctuation, and composition. Clerical, office practices and procedures. Research techniques and report writing. Current MS office software. Filing and recordkeeping. Skill in: Practicing clear and tactful communication with all patients, providers, staff, and administration. Supervisory skills Communicate effectively verbally and in writing and communicate orally in a face-to-face setting and by telephone with public and employees. Establishing and maintaining effective working relationships with employees, other agencies, and the public. Ability to: Demonstrate a professional demeanor and acceptable job performance for all responsibilities supporting established strategic goals and objectives as well as quality measures. Provide concise, applicable, comprehensive, and timely responses to medication queries from patients, providers and the health care team. Keep management informed of key operating issues affecting the pharmacy. Remain current in knowledge required to perform assigned duties. Handle confidential matters and maintain discretion. Abide by all department policies and HIPAA guidelines. Work professionally and effectively with co-workers, clients, and the public. Act with courtesy, tact, and diplomacy. Work Environment: The work environment is associated with an office. The work area is adequately lighted, heated or cooled and ventilated.Employee may be exposed to fumes and chemicals. Physical Requirements: Ability to sit or stand for extended periods of time. Sufficient vision to read printed materials and a computer screen; hearing and speech ability to communicate effectively in-person and telephonically. Capable of frequent repetitive movement of arms, fingers, and wrists.Ability to lift up to 30 pounds.
