Job Summary:
Fountain, Colorado - The organization is seeking a licensed Outpatient Physical Therapist (PT) to provide patient evaluations and develop individualized treatment plans. The position involves completing documentation, ensuring care aligns with physician orders, and participating in professional development activities. Candidates are required to bring their licensure on the first day of assignment.

Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an accredited school, CPR certification, current active licensure for the state of Colorado, 6 months to 1+ year of outpatient clinical experience.
• Preferred Qualifications: Previous assignment with the agency or organization is highly desired, as well as completion of a 6-month outpatient clinical experience.
• Additional Information: Responsible for evaluating patients, developing treatment plans, documentation, participating in student programs, and pursuing ongoing professional growth. Adheres to state practice acts and maintains professional licensure in good standing. Demonstrates understanding of performance improvement methodologies and complies with ethical standards in all professional conduct.
