Licensed Social Worker - Winchester, TN - Heritage Place Care & Rehabilitation

Job Summary:
As a Licensed Social Worker at Heritage Place Care & Rehabilitation, you will play a vital role in providing high-quality care and support to our residents. We are seeking a dedicated and compassionate professional to join our interdisciplinary team and contribute to the overall well-being of our patients. In this full-time position, you will work closely with our healthcare team to assess, plan, and implement interventions to meet the social, emotional, and psychological needs of our residents.

Responsibilities:

• Conduct social work assessments, develop and implement individualized plans, and provide support to residents and their families

• Identify and address social, emotional, and psychological issues that may impact a resident's ability to fully participate in their care plan

• Collaborate with the interdisciplinary team to develop and implement comprehensive care plans that meet the unique needs of each resident

• Provide counseling, education, and support to residents and their families on topics such as coping with illness, grief, and loss

• Maintain accurate and thorough records of patient assessments, plans, and progress

• Participate in quality improvement initiatives and contribute to the development of policies and procedures that improve patient care

Requirements:

• Bachelors degree in Social Work (BSW) or related field

• Current licensure as a Licensed Social Worker (LSW) in the state of Tennessee

• Minimum 1-2 years of experience in social work or a related field

• Ability to work in a fast-paced, dynamic environment and prioritize tasks accordingly

• Excellent communication and interpersonal skills
