RN - Hospice & Palliative Care Nurse

We are seeking a compassionate and dedicated Hospice Registered Nurse to provide exceptional end-of-life care within home or hospice settings in the Midwest region. This role focuses on delivering patient-centered care that prioritizes dignity, comfort, and quality of life for patients and their families.

Key Responsibilities

• Perform comprehensive assessments of patients' physical, emotional, and spiritual needs.
• Conduct initial physical evaluations and develop individualized care plans in collaboration with a multidisciplinary healthcare team.
• Administer medications and manage symptoms following established protocols.
• Educate and support patients and families about end-of-life care options.
• Coordinate services and resources throughout the hospice process.
• Maintain accurate documentation of patient care and communicate changes in condition to the interdisciplinary group.
• Perform venipuncture or collect lab specimens as ordered.
• Notify pharmacy and patients regarding medications and supplies related to hospice care.
• Keep primary and consulting providers informed of patient status.

Education and Experience

• Graduate of an accredited nursing program with current Registered Nurse licensure valid for practice in the relevant state.
• Previous experience in hospice, palliative, or critical care settings is preferred.
• Completion of hospice philosophy and care training is required.

Requirements

• Current Basic Life Support (BLS) certification by the American Heart Association.
• Completion of Mandatory Reporter training within six months of hire.
• Compliance with all federal, state, and professional licensure and regulatory training requirements.
• Strong oral and written communication skills.
• Basic computer proficiency and ability to perform simple mathematical calculations.
