Responsibilities
- Provide direct nursing care to residents in a long-term care setting under supervision
- Administer prescribed medications and document administration per care plan and policy
- Monitor vital signs, assess resident condition, and report changes to the supervising RN
- Assist residents with activities of daily living (ADLs) including bathing, dressing, and mobility
- Collaborate with the interdisciplinary team; participate in care planning and rounds
- Maintain patient safety, infection control, and accurate charting

Requirements
- Active New York State Licensed Practical Nurse (LPN) license
- BLS certification
- Minimum 2 years of LPN experience in a skilled nursing facility/long-term care setting
- Graduate of an approved practical nursing program
- Ability to work evenings (5x8 schedule) and every other weekend; able to work at least 4 of 8 hours if needed
- Local candidates within 50 miles may be considered with a decreased bill rate
