Physical Therapist

We are seeking a licensed Physical Therapist to deliver personalized, in-home and senior living care to older adults, primarily through our Geriatric House Calls™ model. Key Responsibilities Include Providing one-on-one physical therapy sessions in private residences and senior communities Conducting evaluations, developing individualized treatment plans, and delivering evidence-based therapy to enhance mobility and independence Documenting care accurately within electronic medical records Collaborating with a multidisciplinary team of health professionals Educating patients, caregivers, and staff to promote safety and aging in place Incorporating telehealth and virtual care for greater flexibility and patient access Qualifications Include Valid Physical Therapy license and degree from an accredited program Proficiency in basic computer and EMR systems Current CPR certification Physical Requirements Ability to exert medium physical effort, assist with patient transfers, and perform therapeutic activities Capacity to drive, reposition patients, and wear PPE as needed This role offers flexible employment options, competitive compensation, benefits, and opportunities for professional growth within a supportive, clinician-driven environment. #J-18808-Ljbffr
