Clinical Assessor (RN) - Home Care & PCS
Acentra
Acentra Health is seeking a Clinical Assessor, Mid to join our growing team. This role completes needs-based eligibility determinations for Medicaid-funded personal care services provided in home or community settings and assesses level of care to help individuals remain in their homes. The position requires a valid NC RN license and travel to multiple locations. You will interview families, guide beneficiaries in selecting PCS providers, conduct service plan reviews, and ensure privacy and #J-18808-Ljbffr Acentra
