Manage patient progression of care and ensure timely and appropriate patient discharge.

Review patient records and evaluate patient progress, assessing the necessity for hospitalization and quality of care.

Collaborate with healthcare teams to ensure adherence to utilization management and care management plans.

Review new hospital admissions to develop personalized treatment plans based on patient needs.

Provide information regarding healthcare benefits to patients and/or their families.

Support clinical improvement initiatives by providing quality reviews.
