Job Summary We are seeking a dedicated and compassionate nurse to join our healthcare team as a UAS Assessor. The role is essential in providing high‑quality patient care and conducting in‑person assessments for clients to evaluate their eligibility and ongoing needs. Responsibilities Conduct UAS assessments for clients to determine their level of care and support needs. Utilize the Uniform Assessment System (UAS) tool to document assessment results accurately. Provide compassionate and professional interactions with clients in various community settings. Submit completed assessments within designated timeframes and maintain detailed records. Collaborate with other care providers to ensure a comprehensive assessment process. Attend required training sessions and stay informed of assessment updates. Qualifications Registered Nurse, licensed to practice in New York State. UAS‑NY Certified. Bachelor’s degree in nursing preferred. Experience in home care, case management, or long‑term home care. Demonstrated skills in planning, organizing, and directing the work of others. Experience in intake or an admission team of PACE highly desirable. Proficient with DOH UAS assessment instrument.
