Patient Care CoordinatorAtlantic Medical Management (AMM) is hiring a CMA or RMA to work as our Patient Care Coordinator for the Care Management (CM) Department. AMM is a corporate medical management firm that is dedicated to improving patient care. The Patient Care Coordinator is responsible for coordinating a range of activities/tasks with patients. They will work within the scope of practice, and in concert with the Primary Care Provider, patients, caregivers, family members, and other members of the Care Management team. Local travel may be required. Expand your experience and be apart of our new Care Management Team!Essential Functions:Serve as a liaison among the patient/family, community services, primary care providers, specialists, and other care team members to coordinate servicesIdentify and address barriers that impede health outcomesImplement Care Management interventions per the patient's care planProvide education to patient/family within scope of practice under the direction of the Care Managers- Registered Nurse or Licensed Clinical Social WorkerProcesses referrals from members of the Care Management Team appropriately, accurately, and timely according to our established workflowsDocuments all interactions with patients/others appropriatelySchedules/verifies appropriate medical appointments for the patients as neededMaintains appropriate patient documentation in the Care Management documentation platform, in accordance with organizational policies and proceduresAdhere to AMM, organizational, privacy and security policies alongside HIPAA regulations to ensure that patient and network data are properly safeguardedAttend departmental and corporate meetings, local and regional trainings, or other events as requiredWillingly performs other duties assignedQualifications:Certified Medical Assistant or Registered Medical AssistantMinimum of 2 years of experience in a medical or behavioral health environmentAdditional Skills Required:Excellent verbal communication skills.
