Travel RN Case Manager

Home Health jobs in Denver, CO offer 13-week contracts with 8-hour day shifts, Monday through Friday, and 40 hours per week. You must have a Colorado RN license and experience with IGEA EMR. Your responsibilities include assessing patient needs, developing and implementing care plans, coordinating with healthcare providers, and documenting visits. You will conduct 5-6 visits per day, spending 80% of your time in the field and 20% in the office, focusing on low acuity patients. Mileage reimbursement is provided at $0.72 per mile.
