Key Responsibilities

• Conduct thorough assessments and develop individualized treatment plans based on each patient’s unique needs, including crisis plans and behavioral guidelines.
• Timely complete all required documentation to adhere to regulatory standards and facilitate high-quality patient care.
• Implement evidence-based screening tools in collaboration with the clinical team to ensure effective interventions.
• Cooperate with multi-disciplinary teams to design and implement positive behavior support plans tailored to client needs.
• Actively engage in licensing and accreditation processes to uphold the quality and standards of our services.
• Serve as a vital liaison between clients and relevant service systems, state agencies, and community partners to foster comprehensive care.
• Maintain open channels of communication to meet contract standards while amplifying the mission of Bright Harbor in the community.
• Establish and nurture relationships with local and federal agencies to promote our programs and explore new partnerships and funding avenues.
• Seek opportunities for personal and professional growth by attending conferences, workshops, and statewide meetings related to mental health.

• Perform additional duties as assigned by the Clinic Director to support ongoing operations and patient care initiatives.

Patient Population / Client Focus

You will work primarily with a diverse range of clients experiencing various mental health challenges, including those requiring assistance with emotional health, substance use disorders, and developmental disabilities. Your work will focus on fostering recovery and independence, ensuring that each individual receives tailored support designed to promote their overall wellness and reintegration into the community.

Qualifications

• A Master’s Degree in a mental health field is required.
• Possession of a current and independent Licensed Mental Health Counselor (LMHC) credential.

Preferred Experience

• Prior experience in a community health clinic setting is advantageous.
