This role involves planning, organizing, and coordinating home care services, with a strong emphasis on community health and patient education.Key ResponsibilitiesDevelop and initiate individualized care plans in collaboration with patients and caregiversDeliver physician-ordered nursing services and implement preventive and rehabilitative proceduresEducate patients and caregivers to promote health and independenceIdentify and implement discharge planning needsCoordinate necessary equipment and servicesSupervise LPNs, LVNs, and HHAsQualificationsActive California RN licenseMinimum 1 year of direct patient care experience (home health preferred)Current CPR certificationValid driver’s license and insured, reliable vehicleAbout UsSeaport Scripps Home Health is part of the Pennant Group, a nationwide network with over 300 locations.
