Job OpportunityUp to a $10,000 sign on bonus!40 hrs. per week. Monday - Friday 8:00am - 4:30pm. No weekend requirement but holiday requirement per agency guidelines. Greater NashuaAbout the Job:Admission to HHHC's homecare division.Completes an initial home health assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).Utilizes the nursing process in planning, providing, and evaluating skilled needs.Completes a comprehensive patient assessment that is reflected in accurate OASIS scoring.Develops a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
