Physical Therapist (PT) - Facility & Community-Based | Hightstown, NJ | PRN & Part-Time | $50-$85 Per Visit

Stern at Home Therapy is hiring a Physical Therapist (PT) to provide one-on-one care to patients at an established facility and in community-based settings throughout the Hightstown, NJ area .

Choose between part-time W-2 employment with benefits or a flexible PRN opportunity based on your availability.

This opportunity is ideal for a licensed Physical Therapist looking to build a steady part-time caseload or add flexible PRN visits to an existing schedule. The combination of facility and community patients provides a consistent caseload while allowing you to deliver individualized care in multiple settings.

What You'll Do:

As a Physical Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population at an established facility and in patients' homes throughout the Hightstown area.

Responsibilities include:

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on each patient's functional needs and goals.
• Treat patients at an established facility and in community-based settings.
• Help patients improve mobility, strength, balance, safety, and overall function.
• Manage individualized treatment plans with clinical autonomy.
• Complete, accurate, and timely clinical documentation.
• Communicate with patients, families, physicians, facility staff, and members of the clinical team.
• Coordinate with our administrative team regarding scheduling, patient care, and logistics.
• Manage your caseload based on your availability and patient needs.

Compensation:

Part-Time W-2:

• $65 per follow-up visit.
• $75 per 4-unit evaluation.

PRN:

• $72 per 4-unit follow-up visit.
• $85 per evaluation.

Qualifications:

• An active Physical Therapist (PT) license in New Jersey is required.
• Reliable transportation required.

• Home health or facility-based experience is preferred but not required.
