Travel Registered Nurse Case Manager Job

Overview
Registered Nurse - Case Manager in Everett, WA. Travel assignment starting 06/15/2026 for 13 weeks. Weekly pay range $2,232-$2,293 per week. Guaranteed hours 72 hours per 2-week period. Shift: 5x8 Day shifts. Setting: acute care hospital with a dedicated Case Management team focused on discharge planning and care coordination across a broad patient population.

Responsibilities
- Lead care coordination and discharge planning for patients across age groups, including Neonates, Infants, Pediatrics, Adolescents, Adults, and Geriatrics; perform needs assessments, DME ordering, and plan-of-care development.
- Collaborate with an interdisciplinary team including Social Services to ensure safe transitions to the next level of care and timely discharges.
- Manage a caseload of approximately 25-30 patients; monitor progress, identify barriers to discharge, and document outcomes in the Electronic Health Record (EPIC) per policy.
- Conduct needs assessments and coordinate post-acute services, including arranging appropriate supports and ensuring follow-up as clinically indicated.
- Maintain accurate charting and communication with all care team members; participate in rounding and family education to support care plans.
- May float to related units and between campuses as needed to balance workflow and coverage.

Requirements
- RN licensure required; pending license accepted at submittal.
- BLS certification required.
- Minimum 1 year of acute care experience with discharge planning / care coordination.
- Strong clinical skills in acute hospital care, care coordination, discharge planning, needs assessment/DME ordering, and plan-of-care development.
- Locals are not accepted for this assignment.
- EPIC charting experience preferred; ability to document accurately and promptly.
- Must be able to work 5x8 day shifts, holidays as required, and float to related units as needed.
- Orientation ~30 hours will be provided.
