Pay: $56.14/hr.
Schedule: Mon-Fri, Night Shift On-Call: 5:00pm-8:00am (shift ends Friday AM)
Hours: Guaranteed 40 hours/week
Overview
We’re seeking an experienced Hospice RN Case Manager for a night shift on-call role supporting patients in the Colorado Springs/Woodland Park area. This is an on-site position with field travel and guaranteed full-time hours.
Key Responsibilities
- Perform PRN/focused assessments for high-risk members
- Collaborate with providers to implement individualized care plans
- Identify and reassess appropriate, cost-effective levels of care
- Coordinate care transitions with physicians, social workers, discharge planners, and other stakeholders
- Engage specialty resources as needed to resolve patient needs
- Maintain detailed documentation of clinical/functional outcomes
