Job Description
Title: Registered Nurse - Home Health

Schedule: Shift: M-F until shift is complete. On-call required if requested.

Assignment Details:
- Contract Length: 9 weeks
- Guaranteed Hours: 0 hours

Requirements:
- Current state license as a Registered Nurse
- HCHB and Oasis experience
- 1-2 years of experience as a Registered Nurse in a clinical care setting
- 2+ years of home health experience
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance

Roles & Responsibilities:
- Assess home health and/or hospice patients to identify physical, psychosocial, and environmental needs.
- Develop, implement, document, and evaluate the Plan of Care (POC) for quality and continuity of care.
- Educate patients and families regarding care, diet, medications, and home safety.
- Supervise and provide clinical direction to home health aides and nursing assistants.
- Monitor and manage pain through appropriate techniques.
- Serve as a liaison among patients, families, team members, and healthcare professionals.
