Speech Language Pathologist (PRN) - Ahwatukee Post Acute

Responsibilities Evaluate patients within twenty-four (24) hours, or 1 business day of physician referral. Document reasons for delay if evaluation cannot be completed within 24 hours. Develop effective treatment plans and obtain approval for services from referring physicians. Treat patients per physician treatment plan. Supervise Speech Language Pathologist Assistants in direct patient care and patient-related activities, following state practice act. Communicate with supervisor and other health team members regarding patient progress, problems and plans. Participate in patient care conferences, utilization review meetings and rehabilitation conferences as needed. Participate in in services training program for other staff in the facility. Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation per Speech and Language Pathologist in the Board State Practice Act and governmental and third party payer requirements. Record treatment changes per policy and procedures. Instruct patient families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services. Participate in discharge planning. Act as a clinical preceptor for affiliating CFY/RFP Speech Language Pathologist and student programs. Comply with the Speech Language Pathology and Audiology Bureau State Practice Act and the facility policy and procedures. Supervise CFY - Speech Clinical Fellows, if applicable, and assist nursing with training of Restorative Aides. Maintain positive, effective, and appropriate relationships with patients/residents, families, staff, and professional colleagues. Qualifications Education: Bachelor's degree in Speech Language Pathologist Therapy; Master’s or Doctorate preferred. Proficient in computer skills such as inputting data into programs such as Casamba and Point Click Care. Ability to read technical procedures, policy and procedure manuals, and effectively present information and respond to questions from managers, coworkers, and families. Mathematical skills: apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Reasoning skills: solve practical problems, interpret instructions given in written, oral, diagram or schedule form. Licensed as a Speech Therapist in the state. Must meet continuing education requirements per state practice and maintain a license in good standing with the state board. Physical Demands Standing and/or walking very frequently. Sitting occasionally. Reaching, pushing and pulling very frequently. Talking and/or hearing very frequently. Exerting in excess of 100 pounds of force occasionally, 50 pounds frequently, and 20 pounds constantly to move objects. Climbing, balancing, stooping, kneeling, crouching or crawling occasionally. Close, distance, color, peripheral, and depth perception in vision: ability to adjust focus. Work Environment The noise level in the work environment is usually low to moderate.
