Hospice Liaison

Addus Family company seeks a Hospice Liaison to grow hospice services within the Healthcare & Medical Services industry, supporting the Sales & Business Development team. In this field-based role, you will build and manage referral relationships with hospitals, physicians, and community partners, educating them on hospice eligibility, benefits, and Addus programs. You will coordinate closely with clinical and intake teams to ensure timely, compassionate transitions to care. Responsibilities

• Develop and manage referral relationships with hospitals, physicians, and community partners to grow hospice admissions.
• Educate healthcare professionals and families on Addus hospice services, eligibility, and care options.
• Conduct outreach, presentations, and in service sessions to increase awareness of hospice programs.
• Coordinate with clinical and intake teams to ensure smooth, timely patient transitions to hospice care.
• Track and report sales activity, referral trends, and territory performance using CRM tools.
• Collaborate with operations and marketing to execute territory growth strategies.
• Maintain deep knowledge of hospice regulations, payer requirements, and admission criteria.
• Represent Addus at community events, health fairs, and professional meetings.
• Respond promptly and compassionately to inquiries from patients, families, and providers.
• Uphold Addus values of integrity, compassion, and teamwork in all external relationships.
