Hospice RN - Hospice

Schedule
- Shift: 8-hour days
- Weekends: 1/month

Assignment Details
- Guaranteed Hours: May vary, facility may call off up to one shift per pay period (biweekly)

Requirements
- Minimum 1 year of skilled hospice experience
- Experience with Home Care Home Base (HCHB) required
- Current RN license in the state or compact license
- BLS certification required

Roles & Responsibilities
- Provide nursing care and support to hospice patients in their homes.
- Conduct pain and symptom assessments and manage medication needs.
- Educate and support patients and families regarding end-of-life care.
- Monitor patient conditions and report any changes to the interdisciplinary team.
- Perform intake assessments and pressure ulcer staging/prevention.
- Familiarity with Medicare and state hospice regulations is essential.
